They are both connected to each other with cables to allow this transfer of consciousness to take place. 
 
The piece is showing a glimpse of the possible future that could take place within the simulation if humanity fails to increase their own level of consciousness and they remain in the 3D matrix timeline becoming entirely dependent on technology to survive.
 
Instead of a soul ascending at death the individual instead chooses to upload their consciousness into an AI humanoid robot to carry on experiencing the 3D material world of the simulation controlled by the matrix for eternity.
